Family Camp Applications
Family Camp Application Process
Please read the following information carefully before applying, as our application process has changed.
- Each person from your household who would like to attend Family Camp must submit a separate application form.
- Please pay attention to application descriptions to be sure you are filling out the correct form for yourself and each family member.
- Before beginning your application, be sure to have the following things ready for your reference: photo of each child who will be attending, your doctor’s contact information, your health insurance information, immunization records.
- Immunization records can be shared later, as long as they are received before March 24.
- All applications are due by March 24.
- Applications are not considered complete until all components of the form are filled out (this includes immunization forms for those under the age of 18).
- Families will be notified of their acceptance by April 14 and will then receive further instructions.
Applications for Attendees
First/Primary Adult Participant
- This application is for one adult who is your family’s primary point of contact and will be participating in Family Camp programming.
- One person from each family participating in camp MUST fill out the First/Primary Adult registration form as it collects more information than the other applications.

Secondary Adult Participant
- This application is for adults (18 +) who are part of a family that will be participating in Family Camp programming but are NOT the primary contact in that family.
- Example: the spouse who did not fill out the primary application above, a grandparent.

Child Participant
- This application is for children (age 0-17) who are participating in Family Camp programming.
- One Child Application form should be filled out per child participating in Family Camp programming.
- Do not fill out this form if you are applying to be a Junior Counselor (age 16-17). Those interested in being a junior counselor need to fill out the Junior Counselor application in the following section.
- Staff and Volunteers who will be bringing children to camp to participate should fill out this form for their children.

Junior Counselor
- Junior Counselors who will be attending Family Camp as role models, assisting counselors and lead counselors, and helping facilitate camper color group rotations should fill out this application form.
- All Junior Counselors must be age 16 or 17 at the time of camp.
- Junior Counselors will also go through a screening process to be sure they are a good fit to take on this role for the year.
- Please note: all attendees under the age of 18 must be accompanied by a parent or guardian.

Counselor, Volunteers and Staff Applications
Counselor
- Counselors, including Lead Counselors, who will be attending Family Camp as role models and helping facilitate camper color group rotations must fill out an application form.
- All Counselors must be over the age of 18 at the time of camp.
- Private links for counselor registration will be sent via email. If you are a counselor, volunteer or medical staff and have not received an email with your link to register, please contact Diane Bruno at [email protected].
Staff and Volunteers
- Application Forms must be filled out by all Volunteers and NEHA Staff who will be working or volunteering for the entirety of Family Camp.
- All Medical Staff who are attending Family Camp must also fill out an application.
- Private links for staff and volunteer registration will be sent via email. If you are a counselor, volunteer or medical staff and have not received an email with your link to register, please contact Diane Bruno at [email protected].
